501c3 ORGANIZATION STATUS FORM - Please contact the Club Administrator (email@example.com) for a copy of this form. You will need this to document our non-profit status when opening a bank acccount.
INCIDENT REPORT - This should be completed and filed in the event of an accident or injury to one of your players.
SCHOLARSHIP REQUEST FORM - If you have players who are in need of financial assistance, this form must be completed and submitted to Thunder. All instructions are on the form. Note that each season (fall & spring) requires a separate request. Requests for assistance during the fall season are due on July 15th and for the spring season they are due on January 31st.
TRYOUT APPLICATION - Have several of these with you during tryouts in the event prospective players have not yet registered online. Have them complete the paper application and ask them to subsequently register online at the end of the evening.
TRYOUT NUMBERS - This is a handy file you can either print at home or take to a printer. It provides large printed numbers from 1 to 100 that can be pinned to the player shirts during tryouts.
TRYOUT SIGN IN FORM - This is a worksheet you can use at tryouts in order to keep track of the players.
